Connecting to Resources

The CLEAR Family Program provides individualized navigation and supported connections to:

  • Substance-use treatment and recovery services
  • Behavioral and medical healthcare
  • Harm-reduction services
  • Housing, food, transportation, benefits, and social services
  • We help identify barriers, coordinate referrals, and follow up to determine whether the connection was successful.

Outcome: fewer barriers and more successful connections to appropriate care.
